Hey, you got this far! WELL DONE. BREATHE. It's okay if you don't know what you are doing. It's okay for this to be hard. It's okay to be overwhelmed. YOU are normal! Here are the next steps. There is no right or wrong way to do this, and there is no timeline. Do this your way. Let us help if we can.

Set up Tours

Call facilities in your area for tours and to get information. If you have a contract with a placement agent* give them some ideas of what you are looking for.

Things to ask or take note of when you are touring:

  • Menu: are they things that you/ your loved one will eat?

  • What times are meals served? What about outside of those times?

  • Visiting Hours: Do the hours work for you? What is the policy outside of those hours?

  • Do you have to have an appointment or can you drop by unannounced?

  • What routines do they have at the facility? Do they work with your/your loved one's current routines?

  • How are the other residents interacting? (With each other, with staff, with visitors?)

  • How do they contact you? When will they contact you? What will they let you know about vs. what will they just handle on their own?

  • Take note of smells, cleanliness, grooming of residents, ask to see in locked areas, pretend as if it is you who is moving in, and what things you would really want to know. ​

  • Look up the facility with licensing to know about their current status and any violations or complaints filed.  (I added links below to help)

*Placement agents are a great resource to help walk you through the process of finding the right place for you or your loved one.  We are happy to work with or without a placement agent. If you need any recommendations for placement agents before you pick one, feel free to ask us. 

Pre-Appraisal & LIC 602

paperwork and appointments

  • Schedule an appointment with PCP (Primary Care Physician) for a 602 form to be filled out. This is a form required by the state of California prior to moving into ANY facility.  It details the different needs and abilities a person has. If the resident has a dementia diagnosis the 602 form will need to be updated yearly.  We also recommend getting an updated POLST form at this appointment if you don't currently have one. â€‹â€‹â€‹

  • Schedule a preappraisal with any facility you are really interested in.  Some facilities will charge a fee for this so be aware going into it. We do not charge a preappraisal fee. Please just ask! We are more than happy to come meet you and your loved ones to figure out if our care home is the right one.

Put down Deposit

Make sure that the choice you make is the right fit for everyone. We would rather you not pick us and be happy than pick us and not be happy.​

It needs to work for everyone. 

  • Have you done your due diligence? 

  • Do you feel comfortable with the pricing?

  • Do you have a good idea of what the future (as care needs change) looks like at the place you are choosing?

  • Do you get the right feeling when you walk into it?

  • Have you looked over the contract and REALLY read it? 

If you answered yes to all of the above questions then you should probably put down a deposit and

secure your spot!

We require a $1.000 deposit to hold the room for 13 days​ while you make a decision and we will not rent the room to anyone else.  If you ultimately decide to choose us that $1,000 will apply to your first month's rent. If not

Move In

Sign Contracts and Move In

Move-in day can be a lot of stress. To decrease stress and transfer trauma this can be done in whatever way works best for you and your loved ones.  We can sign contracts the day before. You can move things in before your loved one arrives. We can make sure that their day fits their normal routine as closely as possible.  YOU TELL US! 

Move-in day checklist includes: 

  • Check for $500 move in fee

  • Check for $ amount prorated for the date

  • All medications currently taking and any OTC that are necessary and prescribed.

  • Incontinence Supplies (If applicable)

  • Personal Hygiene Items 

  • Discharge Paperwork (if applicable)

  • Personal Items

  • POLST form

  • Power of Attorney Paperwork (if applicable)

  • Insurance Card- we will take copies of front and back

  • ID card- we will take copies of front and back
